• Payments must be written for the exact amount shown above based on your order and written to "US Department of State" exactly.
• Government fees are paid by check or money order only. The US government does not accept credit card payments for these fees.
• All personal checks must have a pre-printed physical address.
• Please write your Child'sName and Date of Birth on the Memo Line.

	DO: Write the government fee for the exact amount for the passport ordered.
	DO: Make check payable to the order of "U.S. Department of State" exactly without any abbreviations.
	DO: Write a check that has a pre-printed name and physical address on it.
	DO NOT: abbreviate "Department" to "Dept" or any other abbreviation or shorthand.
	DO NOT: write the check for any other amount than shown above for the passport selected.
