- Payments must be written for the exact amount shown above based on your order and written to "US Department of State" exactly.
- Government fees are paid by check or money order only. The US government does not accept credit card payments for these fees.
- All personal checks must have a pre-printed physical address.
- Please write your Child'sName and Date of Birth on the Memo Line.
